I don't know how to make the like complicated designs but I know how to make backgrounds and shirts on t-shirt mode
click designs
click add your design
if you are on t-shirt mode and want to make a t-shirt stay there and follow the instructions, if you want a background or a complicated design click go to expert mode
once there, you can upload your background straight away or download the template (Doesn't work on my computer) to make a unique but complicated design
